If you are in our Use platform, you can download your insurance cards from our site by first proceeding to the "Cards & Coverage" page at the top of your screen.

After opening up the Cards & Coverage page, you should see the different insurance plans you are enrolled in listed on that page. To view and download the insurance card for a particular plan, first click on the plan you would like to view. After clicking on the desired plan, you should see the insurance card we generate on the right-hand side of your page. Above the the insurance card, you should see icons to download or print your card.

Note: if you do not see an insurance card generated for a particular plan, you may need to type in the Member and Group ID information for the plan on the left-hand side of the page. After typing in this information, your insurance card will be generated.
